RN or LPN Team Member for a Family Practice

Family doctor is looking for nurse to support her in providing quality care to patients. Nurse will be responsible for:

• Care coordination of complex care and chronic disease patients
• Collecting and documentation of basic health information
• Measuring and recording vital signs
• Preparing and giving injections and vaccines
• Patient education on management of common medical conditions

Requirements:

• Previous experience with acute care or chronic care patients
• Current College registration
• First Aid and CPR certification
• Second language of Punjabi or Hindi is preferred

Job Type: Part-time
Part-time hours: 24-32 per week

Salary: $28.00-$38.00 per hour
