Your tasks:

• Advice and care for patients: You care for and advise patients on various outpatient treatments, such as infusions, artificial nutrition and wound care.
• Creation and documentation of care plans: You create individual care plans and maintain careful care documentation.
• Therapy agreements with treating physicians: You coordinate the therapy and work closely with the treating physicians.
• Network building: You build relationships with clinics, doctors, retirement homes and social services and maintain them on an ongoing basis.

Your profile:

• Completed vocational training in health and nursing: You have a solid education in the health sector.
• Reliable and structured way of working: You work independently, conscientiously and are well organized.
• Strong communication skills and empathy: You are a strong communicator and have a high degree of empathy.
• Class B driving license: A valid driving license is required.
