Your Opportunity:

The Transition Services – Community Care Coordinator screens, assesses, coordinates, and evaluates the needs of patients and families who cannot function independently in the community. This involves extensive assessment, facilitation, and negotiation to assess the need for an alternate level of care and facilitate referrals for Continuing Care Living options. The Registered Nurse – Transition Coordinator must have excellent knowledge of community agencies/resources in order to facilitate the linkages to appropriate services


As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.17/hour – $42.96/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP).Additional Required Qualifications:

3 years community related experience required e.g. Transition Services, Integrated Home Care (IHC) or Integrated Supportive & Facility Living (ISFL). Must have excellent assessment skills, interpersonal, communication, and organizational skills in order to work with a multi-disciplinary team which includes allied health, physicians, and Nursing staff. Caseloads and daily work can be unpredictable – frequent interruptions and multi-tasking required. Ability to reset priorities on an hourly basis due to challenges i.e. capacity triggers. Physically able to perform assigned tasks which may include walking a minimum of 3kms per shift (including stairs) while carrying resource materials weighing up to 25lbs. Required to perform up to 6 hours of keyboarding (up to 50 wpm) and written documentation per shift while navigating a variety of software programs. Demonstrated familiarity with computer programs required. Must be able to type in pertinent patient information. Demonstrated ability to perform all duties in accordance with related legislation.Preferred Qualifications:

Baccalaureate Degree in Nursing preferred; however, skills, and experience relevant to nursing practice in community sector will be considered. Physically able to transfer clients to assess mobility.
