Employment Status: Temporary Full-Time/Part-Time Contract (6 months)

Work Hours: Monday – Friday, 8:00am to 4:00pm (EST)

Salary: $66.54 – $69.18 per hour (Depending on level of experience)

Location: South Oshawa

We are seeking a dedicated and experienced Nurse Practitioner who has experience working on a Geriatric unit, on a Regional Geriatric Program (RGP), a Geriatric Assessment & Intervention (GAIN) team, or a Multispecialty Interprofessional Team Memory Clinic (MINT) team

Key Responsibilities:

  • Conducting comprehensive geriatric assessments, including client interviews, physical exams, and ordering diagnostic tests.
  • Providing direct patient care focused on health promotion and safety for frail seniors at home.
  • Developing and implementing care goals and treatment plans for complex cases, in collaboration with the GAIN Community Team.
  • Prescribing medications, ordering treatments, and coordinating care delivery with the healthcare team.
  • Monitoring client goals and outcomes, participating in case discussions, and providing outreach assessment and case management services.
  • Preparing comprehensive reports and maintaining client documentation.
  • Provide education and support to patients and their families regarding aging, health maintenance, and disease prevention.
  • Utilize PS Suites and Ocean Referral software proficiently to manage patient information and referrals.
  • Work closely with the Geriatric unit, Regional Geriatric Program (RGP), or Geriatric Assessment & Intervention (GAIN) team to ensure coordinated and effective care.

Qualifications:

  • Valid Nurse Practitioner license (RN – Extended Class)
  • Proven experience in conducting comprehensive geriatric assessments.
  • Previous experience working on a Geriatric unit, with a Regional Geriatric Program (RGP), or a Geriatric Assessment & Intervention (GAIN) team.
  • Proficiency in using PS Suites and Ocean Referral software.
  • Excellent communication and interpersonal skills, with a compassionate approach to patient care.
  • Ability to work independently as well as part of a multidisciplinary team.
