Licensed Medical Social Worker – Manhattan
Responsibilities:
Experienced social worker needed to provide psychosocial assessment and supportive interventions of hospice patients and families in conjunction with the hospice interdisciplinary team. Services will be provided in SNFs, IPUs, and homes in Queens.
Qualifications:
- LMSW/LCSW in NY State
- 2+ years of SW experience (healthcare) preferred
- Demonstrated experience in discharge planning
- Ability to work with limited supervision
- Strong interpersonal skills
- Collaborative and team-oriented personality
- Ability to work with a diverse patient population
- Bilingual English/Spanish speaking a plus
- Reliable motor vehicle required
- Computer proficiency and previous experience with EMR systems a plus
- Compensation commensurate with credentialing and experience
