General Summary

  • The Homecare Assistant assists the patient in personal care, rehabilitative activities, nutrition, light housekeeping and/or other therapeutic activities as prescribed in the care plan developed by a registered nurse or therapist.

Essential Duties and Responsibilities

  • Follows the plan of care developed by the registered nurse or therapist.
  • Assists the patient with personal care.
  • Assists the patient with medications, ordinarily self-administered, ordered by physicians.
  • Performs rehabilitation functions such as ROM, ambulation, and transfers.
  • Provides meals according to the patient’s nutritional needs.
  • Provides light housekeeping services.
  • Ensures quality patient care and coordination.
  • Observes and reports any changes in the patient’s condition or home environment.
  • Promotes appropriate levels of independence while maintaining the patient’s comfort.
  • Communicates when unable to carry out planned assignments.
  • Follows agency policies and procedures.
  • Provides safe, competent, age-appropriate care.
  • Completes documentation within established time frames.
  • Maintains productivity standards by performing duties in an efficient and effective manner.
  • Attends a minimum of 12 hours of appropriate in-services yearly.
  • Meets commitments, works independently, accepts accountability, handles change, sets personal standards, stays focused under pressure, and meets attendance/punctuality requirements.
  • Adapts to change, is open to new ideas, takes on new responsibilities, handles pressure, adjusts plans to meet changing needs.

Education

  • High school diploma or equivalent preferred


Work Experience

  • Must have completed one (1) year as a health care aide in a hospital, extended care center, or home health. Has the emotional and mental maturity necessary for establishing a good work relationship with patients, families, caregivers, supervisors, and employees of the agency


Certification/License

  • Nurse Assistant certification (CNA) and CPR certification. A valid driver’s license and proof of current vehicle insurance is required. If assigned a company vehicle you will need to obtain a Class E license within two weeks of assignment. You will be required to obtain a Class E driver’s license at your own expense within two weeks of assignment of the agency vehicle. Will need to complete the Homecare Assistant competency evaluation upon hire. BLS through American Heart Association required.

Mental/Physical Requirements

  • Ability to stand and walk for long periods of time and to walk up and down flights of stairs. Ability to receive and express detailed information through oral communications, visual acuity, and the ability to read and understand written directions. Excellent interpersonal and organizational skills. Ability to lift, transfer, and move patient up to 35 pounds and who vary with ability to assist. Ability to safely operate an automobile.


Working Conditions

  • Frequent exposure to communicable disease via blood and body fluids. Frequent exposure to infectious diseases and hostile persons at times. Works with sharps and other potentially biohazardous materials. Variation in environmental temperatures. Exposure to various community environments.
