Your role and responsibilities:

  • Participates in the development, organization and implementation of integrated health systems at a local, regional.
  • Develops new educational programs and standards of practice for the delivery of evidence-based patient care in any setting.
  • Develops collaborative working relationships with other programs, and clinical service units to facilitate and support quality care for seniors and other patient groups.
  • Establishes and maintains liaison with the representatives of each department and community stakeholders to support delivery of evidence-based care within the program.
  • Contributes to the development of the clinical teams who provide care to seniors and other patients by fostering advanced clinical practice.
  • Consults with clinicians and program leaders to improve individual and population-based patient care.
  • Collaborates with stakeholders to develop, implement, and review program policies and procedures to ensure they comply with professional, legal and Island Health standards.
  • Directs care activities as well as plans, implements and evaluates changes in clinical practice.
  • Participates in QA/QI initiatives, and research and evaluation activities including utilization management.
  • Reviews and evaluates clinical tools related to care of the program population and makes recommendations to the Quality Council about their use in Island Health.
  • Functions as a liaison to community and regional hospitals to facilitate the development of collaborative and participative processes.
  • Identifies at-risk populations and assists in developing prevention strategies with other programs.
  • Coordinates and circulates appropriate data for feedback and problem identification and resolution.
  • Participates as a leader and member in a variety of local, provincial and national program-related committees; initiates projects as required.

What you need to apply:

  • A level of education, training and experience equivalent to a Master of Science in Nursing and five (5) to seven (7) years’ experience in delivery of program-related care; leadership roles preferred. Registration with BC College of Nurses and Midwives as a practicing RN registrant. National certification in program area, where required.
